Speak Up! County launches Community Health Needs Assessment

Davidson County Launches 2025 Community Health Needs Assessment Survey
The Davidson County Health Department is excited to announce the launch of the 2025 Community Health Needs Assessment (CHNA). 

The CHNA is a vital process conducted every three years to help leaders identify priorities to drive funding, programming, and policy efforts. A key component of the process is a community survey to capture the voices and perspectives of local residents.

The goal of the survey is to better understand challenges facing our community, determine community needs, and identify ways to work together to find meaningful solutions to those needs. To participate in the survey, you must be 18 years old or older. The survey should take most people no more than 10
minutes to complete. The CHNA survey is now open to all Davidson County residents and can be completed online at: 2025 Community Health Needs Survey.

years.

In addition to the survey, the assessment process also includes a review of secondary data, key informant interviews, and focus group discussions. An advisory committee will review all of the information to identify priority areas and develop a roadmap for local action.

Davidson County Health Department is completing the CHNA process as an active member of the Central Carolina Community Collaborative. About Central Carolina Community Collaborative: The Collaborative’s purpose is to amplify our local community voice through the Community Health Needs Assessment (CHNA) process. The Central Carolina Community Collaborative brings together 6 health systems (27 hospitals), 11 health departments, and 4 United Way agencies. The collaborative work is centered around: aligned CHNA cycle timing, consistent data collection (both primary and secondary standardized elements with county-level customization available), report writing and formatting (providing a standardized template that each county can then customize), and resources to support funding and sustainability.
